How does Xelopes 40mg Injection work?

It works by reducing the amount of acid in the stomach which helps in the relief of acid-related indigestion and heartburn.

Side Effect of Xelopes 40mg Injection

While generally well-tolerated, Xelopes may cause mild and reversible side effects like skin rash, diarrhea, headache, and gastrointestinal reactions. Rare but serious adverse effects include liver enzyme increases, hepatitis, and anaphylactic shock.

• Avoid eating late at night or before bedtime.
• Inform your doctor if you get watery diarrhea, fever, or stomach pain that does not go away.
• Inform your doctor if you do not feel better after taking it for 14 days as you may be suffering from some other problem that needs attention.
• Long-term use of Xelopes can cause weak bones and a deficiency of minerals such as magnesium. Take adequate dietary intake of calcium and magnesium or their supplements as prescribed by your doctor.

Indication of Xelopes 40mg Injection

Xelopes is prescribed for various conditions, including gastric and duodenal ulcers, NSAID-associated ulcers, gastroesophageal reflux disease, acid-related dyspepsia, severe ulcerating reflux esophagitis, prophylaxis of acid aspiration during anesthesia, Zollinger-Ellison syndrome, and Helicobacter pylori-induced peptic ulcers.

Pharmacology

Its active ingredient, omeprazole, inhibits gastric acid secretion by blocking the H+/K+ ATPase enzyme system in gastric parietal cells. This effect begins within an hour of oral administration, peaks within two hours, and lasts up to 72 hours. When discontinued, gastric acid secretion gradually returns over 3 to 5 days.

Dosage & Administration of Xelopes 40mg Injection

Adult Dose:

• Peptic ulcer: 20 or 40 mg/day for 4 weeks (duodenal ulcer) or 8 weeks (gastric ulcer). Maintenance dose: 10-20 mg/day.
• NSAID-associated ulceration: 20 mg once daily.
• Gastro-oesophageal reflux disease (GERD): 20 mg/day for 4-8 weeks, with a maintenance dose of 20 mg/day after healing.
• Zollinger-Ellison syndrome: Initially 60 mg once daily, adjusted as required. Dose range: 20-120 mg/day.
• Prophylaxis of acid aspiration during general anesthesia: 40 mg in the evening and another 40 mg 2-6 hours pre-op.
• Acid-related dyspepsia: 10 or 20 mg once daily for 2-4 weeks.
• H. pylori infection: Various regimens involving 20 mg twice daily or 40 mg once daily, combined with antimicrobial agents, for 7-10 days.

Intravenous Dose:

• For GERD, peptic ulcers, and NSAID-associated ulceration: 40 mg once daily infused over 20-30 minutes or administered slowly over 5 minutes until oral administration is possible.
• For Zollinger-Ellison syndrome: Initially, 60 mg/day, adjusted according to the response. Daily doses >60 mg/day should be given in 2 divided doses.

Child Dose:

• GERD: Varies by weight, from 5 mg once daily for children weighing 5-10 kg to 20 mg once daily for children over 20 kg.
• Erosive esophagitis: Varies by age and weight, from 2.5 mg once daily for infants aged 1 month to 20 mg once daily for children over 20 kg.

Renal Dose: No dosage adjustment is needed for renal impairment.

Xelopes can be administered orally or via intravenous injection or infusion, depending on the patient's needs and circumstances.

Interaction

It's important to consider potential interactions with other medications, such as ketoconazole, diazepam, phenytoin, warfarin, and clarithromycin, among others. For instance, co-administration with clopidogrel should be avoided due to reduced pharmacological activity.

Contraindications

Omeprazole is contraindicated in patients with known hypersensitivity to it.

Pregnant and breastfeeding women should consult their healthcare provider before using Xelopes.

Precautions & Warnings

Before starting treatment with omeprazole, it's crucial to exclude the possibility of gastric malignancy. This medication is not recommended during pregnancy, lactation, or for children under one year old. Monitoring magnesium concentrations is advised before starting treatment and periodically thereafter. While limited data suggest omeprazole may be present in breast milk, there's a lack of clinical data on its effects on breastfed infants or milk production. It's essential to weigh the developmental and health benefits of breastfeeding against the mother's clinical need for therapy and any potential adverse effects on the infant from treatment or the mother's underlying condition.
